Methylphenidate Side Effects: Who Should Avoid It?

  3. Individuals with seizure disorders

    • Lowers seizure threshold in some cases.

    • Risk: Breakthrough seizures.

  4. Patients with glaucoma

    • May increase intraocular pressure.

  5. Those with substance use disorders

    • Being a stimulant, methylphenidate carries misuse and diversion potential.

    • Safer alternatives may be preferred.

  6. Pregnancy and breastfeeding

    • Limited data; potential risks to the fetus or infant.

    • Generally avoided unless benefits outweigh risks.

    • Allergic reactions to methylphenidate or its components.

Practical Considerations for Clinicians and Patients

  • Baseline evaluation: Blood pressure, pulse, cardiac history, and sometimes ECG before initiating.

  • Monitoring: Regular follow-up for growth in children, cardiovascular status, and mental health changes.

  • Individualization: Dosage and formulation (immediate vs. extended release) should be tailored.

  • Shared decision-making: Patients and families should be informed about both benefits and potential side effects.
